Photo/Video Release
I give permission to Rowlett Dance Academy to take photos and/or videos of my child(ren) that may be used for studio purposes. I understand that Rowlett Dance Academy may use such photography and videography of my child with or without name and for any lawful purpose, including for example such purposes as publicity, information, advertising, Facebook, Instagram, and Web & Social Media content.

Sick and/or Ill Student Policy
I understand that my student should stay home if experiencing any of the following: cough, congestions, sore throat, diarrhea, vomiting, fever, or any other sickness-related symptoms. I understand that my student will return to class only with a doctor's permission or when my student no longer shows any symptoms or signs of sickness. I understand that my student may make-up missed classes in another class of the same level or a level lower. Students may NOT make-up a class in a more advanced level. All classes should be made up prior to Spring Break 2025.

Medical Release
I understand that dance is a physical activity that can result in injury and I agree to release Rowlett Dance Academy and its employees of liability in regards to injury while engaging in activities related to classes and usage of the facility. I understand that Rowlett Dance Academy and its employees will seek medical assistance based on severity and need and may do so without consultation of a parent or appointed guardian of my child(ren) in the event that medical assistance is required immediately.

Financial Responsibility
I understand that lessons must be paid for unless written notice is given to the studio at least 2 weeks prior to beginning class. Unenrollment notification given with less than two week's notice will not be refunded. I understand tuition is paid by automatic draft using either my credit card, debit card, or bank account on file the 27th of each month. There is a a $10 late charge every 5 days for unpaid accounts. I understand that my child will be unenrolled if the unpaid balance increases to more than two month's tuition unless arrangements are made with the office in writing.
I understand it will be assumed that all students enrolled in Ballet 1 or higher will be participating in Rowlett Dance Academy's Nutcracker unless the front desk is notified by email by September 30th. I understand it will be assumed my student is participating in the May recital unless the front desk is notified by email by October 31st. If notification is received after these deadlines, and associated fees have already been charged, fees will NOT be refunded.

I understand that all master classes, camps, and intensives are non-refundable. Cancellations and no-shows may be used for studio credit toward other classes or events upon notification to the office.
